Karlskirche was vowed by Emperor Charles VI in 1713, when a plague epidemic swept Vienna, and he dedicated the church to his name saint, Charles Borromeo, a 16th-century cardinal venerated for tending plague victims. The architect Johann Bernhard Fischer von Erlach won the commission and began building in 1716; after his death in 1723 his son Joseph Emanuel completed the church, which was largely finished in 1737.

Karlskirche's two columns are carved with spiral reliefs of scenes from the life of Saint Charles Borromeo, in imitation of Trajan's Column, and its tall dome blends Roman, Byzantine and baroque references. Rottmayr's dome fresco was painted in the late 1720s, and the high altar carries silver candelabras and an ivory crucifix gifted by Emperor Joseph II. The organ's baroque case dates from 1739, with additions of 1847.
